This website exists to let you tell us you want a call. It collects your first name, your phone number, and whatever sentence you choose to write. We use that to call you back. We do not sell it, and we do not send it to anyone who is not helping us run the business.
This website is not where your health records live. If you become a patient, your clinical information is kept in our practice management system, which is separate from this site and covered by a different notice we give you at intake.
Soleus LLC is a private, cash-pay physical therapy practice in Hammonton, New Jersey. You can reach us at info@soleushq.com or (609) 628-9868 with any question about this policy, including a request to delete what we hold about you.
We ask you not to put medical details into the form. Please save those for the phone call. If you send them anyway, we will treat them carefully, but a web form is not a secure medical channel and we would rather you did not use it as one.
To call you back, to answer your question, to work out whether we are the right fit for you, and to understand which advertising is working. Nothing else.
When you give us your phone number, you are asking us to call you about physical therapy with Soleus.
Text messages are separate and optional. We only send them if you tick the box on our form asking us to. Ticking that box is not required to hear from us, and it is not a condition of buying anything. If you do tick it, we may text you about your request and about scheduling. Message frequency varies. Message and data rates may apply.
Reply STOP to any text to stop receiving them, or HELP for help. You can also just tell us on the phone. Stopping texts does not stop us from being able to call you.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent is never shared with any third party. Phone numbers and consent collected for text messaging are used only by Soleus LLC and the messaging provider that carries our texts, and are never sold, rented or passed to anyone else.
We advertise on Meta platforms, which include Facebook and Instagram. Our advertising pages may use a Meta tracking pixel that records that a page was viewed and that a form was submitted. It is set up to send the fact of the event only. It does not send your name, your phone number or anything you wrote in the form.
We do not put advertising or analytics trackers on our booking or patient systems.
You can limit this kind of tracking through your browser settings, through your device's advertising controls, or through your ad preferences inside Facebook and Instagram.

We do not sell your information. We do not rent it, trade it, or hand it to data brokers. We may disclose information if the law requires it, or if we need to protect someone's safety.
If you never become a patient, we keep your enquiry for up to two years so we recognise you if you come back, then we delete it. If you become a patient, your clinical record is kept for as long as New Jersey law requires us to keep it, which is longer.
Ask us and we will tell you what we hold about you, correct it, or delete it. Email info@soleushq.com. If you are a patient, some records we are legally required to retain, and we will tell you plainly which ones.
Soleus is a healthcare provider. Health information created when you become a patient is protected under HIPAA and is governed by our Notice of Privacy Practices, which we give you at intake. That notice, not this page, is the one that covers your medical record. This page covers this website.
Information you send through this website before you become a patient is treated as a business enquiry, not as a medical record.
This site is meant for adults. We do not knowingly collect information from anyone under 13 through this website.
If we change this policy we will change the date at the top. If the change matters, we will say so plainly rather than quietly editing the page.
